Incidents, Injuries & Safety Concerns May Be Reported Below

Visit Safety at UNI for concerns relating to sexual misconduct, harassment, discrimination, retaliation, bias, ethical concerns or criminal activity.
Non-Employee Injury
Examples Include:
- Any person who is a Guest, Visitor, or Vendor of UNI.
- Students who are not working for UNI at the time of their injury.
- Injuries at a sporting event or other event.
University personnel should not, under any circumstances, answer questions or make any statements regarding university responsibility or coverage for injuries. Individuals asking questions should be referred to the Director of Risk Management.
Employee Injury

Examples Include:
- Any UNI employee, including student employees, who are injured while working.
- Any UNI employee, including student employees, who contract an illness because of their job duties.
- Employees who are injured on the job should report the injury to their supervisor, no matter how minor the injury may be. For additional information please visit Human Resource Services.
EH&S must be notified immediately when an employee is killed on the job or suffers a work-related hospitalization, amputation, or loss of an eye.
Safety Concerns
Examples include:
- Occupational safety and health matters.
- Structural building problems.
- Potentially unsafe equipment.
- Trip/fall hazards.
- Other unsafe conditions impacting the campus community.
To report an Auto Accident
To report problems call Motor Pool at 319-273-2610 during office hours, or after office hours, call Public Safety at 319-273-2712. In case of an accident, refer to the information provided in the glove compartment of the vehicle. For vehicle service call 319-273-2869.
